Financial Aid Forms

Some documents for financial aid do require that we see the original item (citizenship documentation and statement of educational intent). For these documents, please come to Student Services in T.2100 to submit your documentation.

For documents that do not require review of an original item (status letters, tax transcripts, etc.) you may submit the documents to our secure electronic dropbox. Fillable PDF forms need to be saved with your complete information prior to being uploaded to the dropbox. For a demonstration on how to use this secure electronic dropbox, view the video.

Please keep in mind that financial aid verification documents do require a signature as opposed to a typed name on the signature line.

Consortium Agreement

A student is allowed to receive financial aid funding for credits taken at another institution (including study abroad) provided there is a consortium agreement between the two institutions and that courses taken at the host institution will apply toward the student's declared program at the home institution.

Host institution - another school where you are taking courses to meet program requirements at your home institution.

Home institution - the school where you plan to complete a particular program and where your financial aid is disbursed.

If Minneapolis College is your home institution, submit the online Consortium Agreement Request eForm. You will be notified of the result within 3-5 business days. Notification will be send to your @go.minneapolis.edu student email account.

Before you register at a host institution

Your consortium agreement request can only be approved if the courses you want to take at another institution are needed toward your current program at Minneapolis College. To ensure your request is approved, you are strongly encouraged to review your interactive degree audit, meet with your academic advisor, and visit Transferology before you register or become financially responsible for the course(s) at the host institution.

Disability Discharge Acknowledgment

If you have had a loan discharged due to Total and Permanent Disability but now would like to take out additional student loans, you must complete this form. Fillable PDF forms need to be saved with your complete information prior to being uploaded to the dropbox.

Education Form

Please complete this Education form if this information is requested on your status letter found in eservices.

Foster Care Verification

A student may be asked to provide verification of prior foster care placement or state ward status. This form can be completed by a social worker or other agency staff to verify foster care placement or state ward status. Fillable PDF forms need to be saved with your complete information prior to being uploaded to the dropbox.

Household Size, Number in College Verification

Please complete this form if you are required to verify your/your parents household size and or the number in college prior to receiving financial aid. Dependent students will need the email of one of your parents listed on the FAFSA to complete this eform.

2024-2025

Identity and Statement of Educational Purpose

This process needs to be completed in person in Student Services. You will need to provide a valid government-issued ID with a photo. If you are unable to provide this documentation in person, please send a message with the reason that you are unable to come to campus to Financial.Aid@minneapolis.edu for your next steps.

How to Request IRS Verification of Non-filing Letter

What is an IRS Verification of Non-filing Letter?

An IRS Verification of Non-filing Letter provides proof that the IRS has no record of a filed Form 1040,1040A or 1040EZ for the year you requested.

Non-Tax filers can request an IRS Verification of Non-filing of their tax return status-- free of charge-- from the IRS in one of 3 ways:

If you have trouble entering your street address into an online form, try these address-matching suggestions.

Online Request

Note: This is typically not available if you have never filed taxes before in prior years. If this is the case,
please use the paper request process detailed below.

  1. Click “Get Transcript ONLINE” (If at any point, you cannot validate your identity – for example, you cannot provide financial verification information or you lack access to a mobile phone – you may use
  2. Get Transcript by MAIL (see Paper Request Form below)
  3. Enter the non-filer’s Social Security Number, email address, filing status, account numbers for loan or credit card associated with your name, and mobile phone associated with your name.
  4. Click “Continue”
  5. Select "Verification of Non-filing Letter” and in the Tax Year field, select "2021".
  6. If successfully validated, you will be able to view your IRS Verification of Non-filing Letter.

Telephone Request

Note: This is typically not available if you have never filed taxes before in prior years. If this is the case, please use the paper request process detailed below.

  1. Non-filers must follow prompts to enter their social security number and the numbers in their street address. These numbers must match the SSN and address as they appear on the non-filer’s most recent form 1040. (Problems entering your street address? See Paper Request Form below)
  2. Select "Option 2" to request an IRS Verification of Non-filing Letter and then enter "2021".
  3. If successfully validated, non-filers can expect to receive a paper IRS Verification of Non-filing Letter at the address provided in their telephone request within 5 to 10 days from the time of the request.
  4. IRS Verification of Non-filing Letter requested by telephone may be sent only to the non-filer; the IRS will not send non-filing verification to a third party.

Paper Request Form – IRS Form 4506-T

  1. Complete lines 1 – 4, following the instructions on page 2 of the form.
  2. Line 3: enter the non-filer’s street address and zip or postal code. Use the address currently on file with the IRS. (Problems entering your street address?)
  3. Line 5 provides non-filers with the option to have their IRS Verification of Non-filing Letter mailed directly to a third party by the IRS. Do not have your IRS Verification of Non-filing Letter sent directly to Minneapolis College.
  4. Line 7: Select the checkbox on the right hand side for Verification of Non-filing.
  5. Line 9: Year or period requested field, enter "12/31/2021".
  6. The non-filer must sign and date the form and enter their telephone number. Only one signature is required when requesting a joint IRS Verification of Non-filing Letter.
  7. Mail or fax the completed IRS Form 4506-T to the address (or FAX number) provided on page 2 of Form 4506-T.
  8. If the 4506-T information is validated successfully, non-filers can expect to receive a paper IRS
  9. Verification of Non-filing Letter at the address provided on their request within 5 to 10 days.
